Students are being sent home a packet which includes health forms that need to be filled out for the new school year. This year you have the option to fill out the health forms online though the Power School Parent Portal.
If you do not have a parent portal account, please contact your school’s main office and they can help you set up an account.
To fill out the forms online:
1. Login through the PowerSchool Parent Portal Account
2. Select “forms” on the left hand side toolbar
3. Complete each form by selecting them individually and clicking on the submit button
The forms can only be accessed by a laptop or desktop computer. You cannot complete the forms via the PowerSchool app on your phone. For questions, please email familysupports@napsk12.org.
